Please read closely, as we've recently made changes to the way patients access their medical records.

For Patients with Appointments After 6/21/16

We have transitioned to a new patient portal that allows you to access your medical records, contact your physician, schedule appointments and more.

To access the new patient portal and medical records from appointments after 6/21/16, launch Athena Health.

 

 


For Patients Regarding Appointments Prior to 6/21/16

To access any records created during appointments prior to 6/21/16, please use our old patient patient portal, NextGen (NextMD). 

 
Important Notice
 
Please note access to NextGen will only be available through July 21st, 2016.
 
If you feel you need historical health information from the NextMD portal, you should download and save it as the information will not be available after July 21.
 
After July 21st, you'll need to reach out to your physician's office to get access to any medical records from appointments prior to 6/21/16.
